My votes are tied between Ralph Nielsen's "Scream" and Adrian Lloyd's "Lorna" In 1962 "The Scream" must have been like musical waterboarding to any kid's parents and the guy in the booth recording it must have lost his mind when he saw the VU meters on the board peg in the red for the entire song.

Is there any other insanity like this out there?

Let's not forget this nugget. I apologize It's not garage but no less insane especially when you consider it was recorded on Pat Boones DOT label.
I had a stone mint copy with mint sleeve 10-15 years ago. I think I sold it to Tim Warren. Killer rekkid! Somewhere on facebook someone was selling a clean copy yesterday, with sleeve

Far, far away from the comfort of the garage world, give this pair of lunatic/spazzed out tunes by maestro Hezie Johnson a listen:

Craziness checklist:
Atonal vocals
Bizzarro lyrics
Lack of musicianship
No sense of rhythm or timing
Warning - you can't unlisten these - they'll be seared into your gray matter for eternity if you dare giving them a spin.
