Came across this during a record-hunting expedition on Saturday...

Both sides have white labels with "DJ Copy - Not For Sale" stamps.

A-side deadwax: U-41674M-A, SJW-9346

B-side deadwax: U-41674M-B, SJW-9346

Any idea when and how this was issued?

1970s bootleg / unauthorized pressing
They retained the original Wakefield sequential master number found on the original pressing (SJW+ number), but the other master number is a United Records mastering / pressing from the 1974/75 time frame.
I used to see these around for $5-10. Obviously created for the Alice Cooper fans / collectors.
