Triangle crack on vinyl

Once I received a 45 in the mail with a serious crack in the shape of a small triangle at the edge of the record, extending a little less than half an inch into the vinyl. Recently I found another record with a similar triangular indentation, but not yet cracked through. Odd to see this kind of flaw twice.

Could this be a mechanical indentation from shifting the pressed record out of a machine?
 
I've never seen that type of damage, however, I've seen some records that look like someone put out a cigarette at the beginning groove area ; A burn mark that make a bad noise. Turns out it was a reaction to the acid in the brown paper sleeves they used in the sixties. I saw a batch of 45s that were in those brown sleeves that came from the Crup stash years ago. Oddly, about one in ten of the same record titles were stuck to the sleeves at one small area. When I gently pulled the sleeve from the vinyl, there was the burn mark.
